Short, one-word descriptions are to be avoided.
Please proof-read before submitting the workshop.
Either Doreen or Res will approve the workshop and forward it to Maki.
You do
not
need to give Doreen a printed copy of this lesson plan.
Correct and timely completion of this form is part of the "Additional Hour Program". Failure to submit it by the deadline may disqualify you from the weekly benefit.
